How clinical authority works at Tides. Every intake gets reviewed before any compound is dispensed. If a protocol fits the patient's goals and health context, Dr. Rachel signs off and the order ships. If it doesn't, the order is declined and the card authorization is released without a charge.
Refills carry the same gate. When a patient clicks Refill in their portal, the card is authorized but not charged until Dr. Rachel signs off on the cycle. Every fourth refill is held for a direct check-in. Required lab panels (A1C and lipid for GLP-3, for example) are requested through the patient's portal and reviewed before continuing the protocol.
Operational tools assist — order routing, structured intake summaries, protocol templates — but the clinical decisions stay with Dr. Rachel. When AI eventually drafts a check-in question or a stack recommendation, it's reviewed and edited by her before it ever reaches the patient. Rachel-gated, not Rachel-replaced.
